so far our fight against corona has been reasonably good, although new cases are coming regularly with doubling rate of around 12-13 days. A major worry is a second peak of new cases after lock down. if we follow the same rules & precautions in post lock down period then we can certainly prevent the rise in new cases. Also we need to develop new working style and get adapted to corona. social distancing, frequent hand wash, wearing mask, avoid touching your face area,avoid travel and social gatherings. we have to be very careful till we get the vaccine. corona pandemic-

corona pandemic has shattered the world.It has disturbed us mentally more then physically. we are almost in stage 3 of the transmission now. our plus points are a large younger population,rising temperature & reasonably good herd immunity. Negative points are not following strict isolation,poor hygine and densly populated country. what ever we are couragious people with a fighting spirit and we will certainly win the fight against corona.

This pandemic has re-united us.A two weeks lockdown has shown the path to live. we again have realised the value of family. we are learning to be happy with minimum available resources. we have learnt that fuel has no value if not consumed. our atmosphere is less polluted. In fact nature seems to be repairing its ozone layer.

so be positive. we are going to defeat corona and in the process we are learning to live simply and happily and should take a pledge not to mess up with nature …

 

UNILATERAL VISION LOSS

Loss of vision in one eye may be sudden or slowly progressive. In the later case it is often detected late. with sudden loss most of the patients lands up with ophthalmologist ,presuming that they have a cataract or their spectacle needs to be changed. let me tell you that neither of these cause a sudden visual loss. In fact only a few opthalmic conditions like retinal detachment or angle closure glaucoma known as KANCHBIND or hemorrhage in eye leads to sudden visual loss & of course, these conditions needs prompt attention & treatment.

Neurological causes of sudden vision loss are more dangerous & disabling and at times when the vision recovers promptly with in minutes & the patient doesn’t take this episode seriously. This transient loss also known as amurosis is due to sudden cut off in the blood supply to optic nerve and or retina and if not intervened urgently may lead to permanent loss of vision. Another pattern is impaired field of vision which is due to brain stroke and is often not realized early by the patient but is an emergency & needs prompt & early intervention.

Involvement of optic nerve(which carries visual sensation to brain) may start suddenly & progress over to days. It often leads to eye pain on movement & early impaired perception for red color. Also patients may experience bright dots or flashes of light in front of the eye. The optic nerve is affected in many conditions & if not diagnosed or treated early may lead to permanent vision loss in one eye & may sequentially involve other eye.

Diabetes , high blood pressure, raised lipids , chronic joint diseases , autoimmune disorders , excessive smoking & alcohol , certain medicines , exposure to some toxins & nutritional deficiencies all can affect the optic nerve , retina & their blood supply leading to profound & permanent vision loss.

so friends never take vision impairment lightly particularly if sudden , progressive , accompanied by pain in or around eye , & if you have any of the above mentioned risk factors.

migraine headache-do’s and dont’s

if you suffer from severe throbbing pain on one side of your head with nausea,vomiting,dislike for light or sound and which lasts for few hours to a day and after which you feel exhausted or drained out  then you have migraine. It usually run in families. Migraine attacks may start during school age and persist till adulthood. It is more common in women. The frequency of attacks varies from person to person.  The common triggers for migraine are lack of sleep, fasting, low water intake, excessive stress or fatigue, over exposure to sunlight, chocolates, cheese and junk food.   Avoiding these triggers may protect you from migraine attack.

To abort an acute attack you may take analgesics alone or in combination with tryptans either oral or injectable. Try to sleep in dark environment although difficult. But remember that too frequent use of analgesics may worsen your migraine. Long term treatment (preventive) is given for 3-5 months to those who have frequent attacks. Some serious brain conditions may mimic migraine, hence don’t self medicate and take treatment under proper medical supervision.

Drinking plenty of water, avoiding junk food, proper rest and exercise, learn to release stress and refrain from analgesic abuse are some of the ways you can prevent migraine.

Remember that migraine is not a disease but just a headache. Although it is worse then any disease for the sufferers. But avoiding triggers and adopting simple measures along with proper treatment can keep you headache free.

 

 

Ischemic stroke in adults – World Stroke Day

Stroke or brain attack is due to transient cut off in blood supply to the brain due to a clot in the blood vessels supplying brain, the clot is formed locally or dislodged from heart and large blood vessels in neck. Age, high blood pressure diabetes, smoking, central obesity, sedentary life style and high cholesterol are major risk factors for stroke.
Stroke presents with sudden weakness in a limb or on one side, speech changes, vertigo & imbalance, vision impairment and behavioural changes. Sometimes these symptoms may last for few minutes and the patient is normal. This is transient ischemic attack and is a harbinger of major stroke in next 48hrs. And hence should not be neglected.
First few hours after the onset of stroke are most important & we call it GOLDEN HOURS. As this is the time when prompt intervention can reverse brain injury and improve outcome in these patients. We inject a drug called tPA with in 4.5 hrs. Of onset to dissolve the clot and restore the blood supply to the brain, patients who receive this injection within one hour of onset of stroke have the best outcome in terms of quality of life.
Even if you are late for injection therapy, don’t get upset. These days we are using endovascular interventions i.e. Remove the clot by mechanical devices upto 6hrs from onset of stroke.
Remember that stroke may be devastating and make a person handicap for the rest of life. The lack of awareness and the nihilistic approach are the major reasons for failure to receive clot busting therapy in time and hence poor outcome.

Low backache has a life time prevalence of 80%. It is just second to common cold for abstinence at work. Majority are mechanical pains related to faulty postures & sedentary life styles. They don’t need any special tests or treatment. It subsides in few days to weeks in majority. Avoid over use of pain killers.

Do not refrain from  physical or day to day activities. Remain active. Some back aches are serious and requires prompt medical attention.

Warning signs for serious low back pains-

  • Continuous severe pain not relieved by rest and pain killers.
  • Awakens you in night & worsen after lying down.
  • Associate with fever / weight loss.
  • After severe trauma.

The best way to avoid back aches is regular exercises that strengthen your back and abdominal muscles. Also learn to adopt healthy postures while sitting, standing, bending forward and lifting objects from ground.
